An inhaler does not work empty: the cotton wick holds the liquid and lets the vapour through. Without it, the product spills and the object is useless.
A wick is replaced when it no longer smells of anything after being soaked again, or when it has packed down. Cotton eventually loads up with residue and holds less well.
